BEACH PERMIT APPLICATION FORM
• Beach permits relate to Spring Lake Park’s “swimming facility” which is defined as the lake and sand
beach.
• Groups of 10 – 30 persons are required to obtain a beach permit to reserve space at the “swimming
facility.” Groups larger than 30 persons are not permitted.
• The “swimming facility” is not reserved exclusively under an issued beach permit.
• Unforeseen circumstances beyond the control of the Village may cause closure of the beach.
• Beach permits will not be issued for holidays. Weekend permits are restricted 11 a.m. – 5 p.m.
• One adult 21 years of age or over is required to supervise the group. At all times during the event, the
permit must be in the actual physical possession of the supervisor. Upon request of any Village official,
park attendant, or Lincolnshire police officer, the permit shall be produced and exhibited.
• The $10.00 beach permit application fee is due with submission of this form.
• Beach tags (season tags or daily wristbands) for each member of the group require additional
purchase.
• This application form must be submitted at least 10 business days prior to the event.

As the applicant, I understand I am responsible for the group. I have read the above and attached,
along with the Spring Lake Beach Rules and Section 8-1 of the Lincolnshire Village Code related to
Village parks and agree to comply with the all rules and regulations. I understand any damages and/or
expenses incurred by the Village related to the event will be my responsibility.
