Readmission application materials must be typed and hand carried or emailed to the PTA Program
Coordinator at jdyett@bccc.edu; handwritten applications will not be accepted.
YOUR RE-ADMISSION APPLICATION PACKET MUST BE COMPLETE WITH ALL THE REQUIRED
DOCUMENTATION BEFORE YOUR SUBMISSION MATERIALS WILL BE CONSIDERED. PLEASE
SUBMIT THIS COMPLETED READMISSION APPLICATION BY MARCH 27, 2020, (for students who were
unsuccessful in the Fall 2019 semester) or BY JULY 10, 2020, (for students who were unsuccessful in Spring, 2020).

I have completed the PTA program’s re-admission application, and have included it as a part of this
application packet.
Official Transcript Submission: I have attached ALL official sealed transcripts to this
packet from ALL previously attended colleges/universities. (Please be sure to only submit
transcripts with applicable courses for the PTA program, if additional courses were taken at
another institution after the last PTA course was taken, otherwise mark N/A in the box.)
I have attached a copy of my BCCC student inquiry report, and have included it as a part of this
readmission Application packet.
My student inquiry report notes that I am in good academic standing. (Students on academic
warning are not eligible for readmission.
I understand that my re-admission application will not be accepted by mail and all PTA program
re-admission application packets must be hand carried and submitted to the PTA Program
Coordinator. I understand that the processing of my PTA application cannot be
completed until receipt of all indicated documentation is present in my file, including my official
sealed transcript (s) from all schools by the issuing authority.
