Required Application Materials:
• Official transcripts from any colleges or universities attended, if not provided for your application to USU Eastern. If
the documents are not in English, a certified English language translation is also required.
• Evidence of English proficiency if you have taken less than 48 credits at USU Eastern. Official TOEFL and IELTS scores
submitted to the USU Logan campus by the testing agency at the address below, or by passing level 4 of the Intensive
English Language Institute (IELI) placement exam.
Documents required to process your new I-20 or DS-2019:
• Affidavit of Financial Support form, completed and signed by your sponsor.
• Bank Statement, on bank letterhead or certified by a bank official.
• Copy of your passport, visa, and current I-20.
NOTE: There is no transfer application fee; however, shipping fees will be charged for any I-20 that must be mailed.
