Have a GPA of 2.0 or higher (attach transcript)
Have completed 45 credit hours (attach transcript)
Have attended a BSW Orientation – Date:
Have completed a BSW Conduct Agreement / Criminal History Disclaimer form
Have turned in a Personal Narrative
Have submitted both references
Basic Skills classes completed with a grade of “C” or better (attach transcript):

Math 111 (3), College Algebra -or- Math 131 (3), Contemporary Math – Semester:
