2) Obtain (print) a student (unofficial copy) of your transcript from the BPCC website (LOLA).
3) Locate each attempt of the courses listed on this form and highlight them on the unofficial transcript. Attach it
to the back of this form.
4) All prerequisite courses must be successfully completed by the end of the spring semester of your application
year. If you are currently enrolled in courses that you will not finish until the end of the spring semester, place
“IP” in the box for the grade indicating that these courses are still in progress.
5) All courses listed above must be completed with a C or higher prior to consider for enrollment in the OTA
Program.
