∆ A completed SBAC Soror Report Card must be submitted to be eligible for
Chapter Awards. See Policies and Procedures for Chapter Awards
∆ Submit to Membership Services at the April Chapter Meeting
∆ Activities will be verified by Membership Services Committee utilizing PPD
Reports, End of Activity Reports, Financial Reports and sign-in sheets
∆ Do not alter the Report Card layout (One event per box)
∆ See Chapter Calendar for corresponding Activities’ Legend
∆ Committee meetings may not be substituted for activities
∆ Activities may not be substituted for Committee meetings
∆ Delta Sigma Theta State, Regional and National events include: National
Convention, Regional Conference, State Cluster, DDAC and DDNC
∆ Combined activities, events, meetings and/or donations can only be
utilized to satisfy one report card entry
