Vendor Attestation
Upon request of the Arizona Peace Officer Sta
ndards and Training Board,
will provide the lesson plan or other information sufficient to determine compliance with
AZPOST rules.
I/we agree to provide documentation that demonstrates the expertise and ability of the
instructor to enhance peace officer training in a specific field.
I/we agree to maintain student attendance rosters for three years from the date of the course
completion.
I/we understand the Program Compliance Confirmation form approving AZPOST credit, will
expire on the date listed on the document.
I/we understand a new course review packet must be submitted to AZPOST if the
program/lesson plan changes or prior to the expiration date to ensure continued credit
authorization.
I/we agree to all Distance Learning requirements set forth in AZPOST Policy 02-11.
I/we agree to distribute the Program Compliance Confirmation forms to attending officers at
the completion of the class.
I/we understand AZPOST does not “certify” training; therefore, all advertising may only say,
“This course has been determined to qualify for AZPOST continuing training credit.”
I/we understand we are responsible for our own course advertisement.
