The Enhancing Excellence in Teaching Program (EETP) provides forgivable loans to teachers who are enrolled in
an eligible graduate program at an eligible Nebraska institution. Prior to completing this application, please
review the program information, including eligibility criteria, program requirements, and fund disbursements,
on the Nebraska Teacher Education web site at:

This application may be completed on paper or online and printed. Save a copy of the completed application
for your records.

Categories 1, 3, 4 and 5 require the completion of a graduate degree program.

1. Shortage Area. Graduate degree program in a shortage area, as determined by the Nebraska
Department of Education, at the time of the current award period.

Nebraska Department of Education, at the time of the current award period.

3. Curriculum and Instruction. Graduate degree program resulting in a degree in curriculum and
instruction.
4. Advanced Degree. Graduate degree program in the subject area for which a teaching endorsement is
already held.

5. Endorsement Requested by School Administrator. Graduate degree program for a teaching
endorsement, not considered a shortage area, as requested by the administrator of the school district,
school, or Educational Service Unit at which you are employed.
A statement, signed by the administrator of the school district, school, or Educational Service Unit at
which you are employed, which provides support that the graduate degree program in which you are
enrolled will result in a teaching endorsement that will be beneficial to the students of the school
district or school must be submitted with this application.

FINANCIAL REQUEST
Enhancing Excellence in Education Program awards may be applied for on an annual basis in an amount of not more
than $175 per credit hour, up to a maximum of $3000 for 18 credit hours. Eligible coursework includes classes that
have been identified by the institution in which you are enrolled as part of your graduate program and that begin
during the award period (July 1, 2022 June 30, 2023).

I hereby attest that the information on this application and any related documents is true, complete and
accurate. I acknowledge that if my application is approved, I must enter into a contract with the Nebraska
Department of Education prior to receiving any funds. I certify that I intend to complete the eligible graduate
program on which my eligibility for this award is based and maintain certification and employment as a full-
time teacher in Nebraska for the time period equal to the number of years required for award forgiveness. If I
fail to meet the requirements of award forgiveness pursuant to Section 79-8,137.04 R.R.S., the award becomes a
loan and I agree to repay the loan in accordance with the laws and regulations of the Excellence in Teaching
Act.

The completed application and all supporting documentation are to be submitted to the designated contact person at the
institution in which you are enrolled per instructions on their EETP information page accessed from the EETP Application
Information webpage at:

Applications submitted for consideration of an EETP loan will undergo an initial review by the institution in which you are
enrolled and will then be forwarded to the Nebraska Department of Education (NDE) for final review and approval.
