As per Academic Regulation 7g, students may appeal an academic suspension in cases of health issues, bereavement
and/or other acceptable cause, duly authenticated. As part of the suspension appeal you must submit the following along
with this form:
1.
A letter to address the following questions:

Outline the reasons for your academic performance to date.
B.
What specific steps have you taken to address any issues identified in Question A.
C.
What are your plans to ensure academic success in the future.
2.
Attach relevant professional and other documentation to support your request.
3.
Return the completed form along with supporting documentation to your Faculty Advising Office.

Appeal approval is not guaranteed and your current suspension remains in place during the appeal process.
Once a decision has been reached by the Appeals Review Committee, you will be notified via “Banner” email. Please
note the review process can take up to 30 days to complete. A copy of your appeal will be kept in your student file.
