INSTRUCTIONS (Please read thoroughly before meeting with your academic advisor.)

proposed courses will count toward your WSU degree. It is recommended that you select multiple courses for potential enrollment, should certain courses not be

to study abroad. At the time of enrollment at your host institution, you may enroll in courses not listed on this form. In this case, you must re-submit this form
(signed), listing the additional courses in which you are actually enrolled, to your Academic Advisor, or a designated representative in the Academic Department
or College from which you are requesting credit, for approval and determination of equivalency. Your transcript will not be processed unless the form is re-
submitted with the additional courses.
ADVISORS: Please review the instructions for students listed above. If the student is attending one of WSU’s exchange partner universities, then the courses may
have already been evaluated. You will need to check Banner (SHATATR) for transfer course equivalencies regardless whether the student will study at an
ust be evaluated by the academic
department/college in which the course would be housed. You should first list all of the courses that have already been evaluated and are listed in Banner with the
transfer equivalencies on this form. Then, list courses which will need to be evaluated. Please direct the student to the appropriate departments for evaluation if
you are not the person who can evaluate them. For all language courses, please direct students to the MCLL undergraduate coordinator.

department (ex. COMM 2000 = lower division elective in Communications; HIST 4000 = upper division elective in History)
