1. NOTICE IS GIVEN THAT (name of petitioner)
has filed a petition to change the name of your child or the names of your children.
2. You may refer to the filed petition for further particulars. (All of the case documents filed with the court are available

4. If you fail to attend the hearing and inform the Court why the petition should not be granted, the court may grant the
petition.
5. Pursuant to California Code of Civil Procedure Section 1277, all persons interested in this matter are directed to
make known any objection that they may have to the granting of the petition for change of name by filing a written
objection, which includes the reasons for the objection, with the court at least two court days before the matter is
scheduled to be heard and by appearing in court at the hearing to show cause why the petition for change of name
should not be granted. If no written objection is timely filed, the court may grant the petition without a hearing.

Note: 30 days or more before the hearing, the non-petitioning parent must be personally served pursuant to
Code of Civil Procedure Section 415.10 or served by mail pursuant to Code of Civil Procedure Section
415.40 with a copy of this Notice of Hearing or with a copy of the Order to Show Cause for Change of Name.
The parent may be served by mail only if the parent is outside of California.
