1. That this affidavit is made pursuant to the provisions of the City of Kannapolis
Ordinance Section 10-292 to induce the Chief of Police to issue a Parade Permit.
2. That the information contained in my application for a parade permit is complete,
true and correct to the best of my information and belief.
3. That I have received a copy of the City ordinance governing parades, and that I
agree to fully comply with any and all provisions therein.
4. That if I am acting on behalf of the head of a sponsoring organization or group, I
have been authorized in writing to apply for a parade permit on behalf of that
organization or group.
5. That the undersigned applicant agrees to release and save the City of Kannapolis
and its officers and employees for any claims, actions, and lawsuits, arising out of the
conduct of the parade, excepting injuries or damages proximately caused by the City of
Kannapolis.
