The applicant affirms on oath that the information provided b
y the Applicant to the City of La
Porte herein is true and correct. Further, Applicant understands his or its obligation to keep the
information contained in this application current, and promises to file any additions or corrections
to the information contained herein promptly with the City Secretary of the City of La Porte.
Applicant further understands that failure to truthfully provide the information required herein and
keeps that information required herein current and updated is grounds for revocation of this license.
The Application further understands that his or its signature upon this application authorizes the
City Secretary of the City of La Porte to cause an investigation to be made of the Applicant’s
business responsibility or moral character, and that if the results of this investigation prove to be
unsatisfactory to the City Secretary of the City of La Porte, this application shall be denied.
Applicant further understands that before a license may be issued to Applicant for the conduct of
Applicant ‘s business within the City of La Porte, a bond shall be filed by the Applicant with the
City Secretary of the City of La Porte, said bond to be executed in favor of the City of La Porte by
the Applicant, as principal, with two (2)sureties upon which service of process may be made in the
State of Texas, said bond to the approved by the City Attorney, all as provided in Ordinance
No.1356 of the code of Ordinances of the City of La Porte.
Further, Applicant represents that Ordinance No. 1356 of the City of La Porte has been read in full
by Applicant, and that Applicant understands the provisions of same, particularly regarding said
Ordinance’s prohibition of certain practices, said Ordinance’s disclosure requirements, the
licensure, the license display, and license application requirements, and said Ordinance’s bonding
requirements.
It is expressly understood by Applicant that this license, if granted by the City of La Porte, is non-
negotiable and non-transferable. This license is not exclusive, and the City of La Porte has explicit
authority under said Ordinance No.1356 to grant licenses to any applicant that meets the
qualifications as set forth in said Ordinance.
This license shall continue until the ______day of _______20, _____. In any event, the City of La
Porte reserves the right to cancel this license at any time, upon giving applicant licensee
________days notice of said cancellation.
A Licensee acknowledges that it enters upon the operation of this license, if granted assuming sole
and entire responsibility for any loss of life or injuries, of property damage, that may be sustained
in connection therewith; Applicant Licensee agrees to indemnify Licensor City of La Porte, and to
hold Licensor City of La Porte, harmless in regard to any and all such liability.

This is a business with the same basic objective as any other business, to make money. Our products are sold
door–to–door by young vendors that we call teenage salesmen.
A teenage salesman is an independent contractor. He or She receives merchandise on consignment and at regular
intervals pays for what he/she has sold. A teenage salesperson always deducts his/her own commissions before
paying for the merchandise that he has sold. He can return any unsold merchandise at any time for full credit to
his account.
A teenage salesperson‘s earnings depend upon his/her ability as a salesperson and the time and effort that he puts
forth. As a teenage salesperson, a youngster receives a good commission (at least 20%) per sale on each item that
he/she sells. He/She is paid strictly on a commission basis. There are also different sales incentives that teenage
salesperson can work toward. These include cash bonuses and activities.
A teenage salesperson will work with a group of teenage salesman that travels under adult supervision to
different areas to work.
Parent please be advised. This teenager is neither an agent or employee of any company but strictly an
independent contractor.
I have read the information on this agreement and as the parent or legal guardian of the youngster listed above, I
am giving my permission for him to become a teenage salesperson. I also agree to hold supervisor and all related
parties harmless in the event of accidental illness or injury.
In case of an accident or sickness when my youngster is selling is selling while under the adult supervisor or on
an activity, I am giving the adult supervision my permission to proceed with whatever medical attention may be
deemed necessary .I have written on the back of this sheet any special medical or physical conditions about my
youngster that those working with him should be aware of.
