UNDER ILLINOIS SUPREME COURT RULE 711(g)
Instructions: After completing Sections A, B and (if applicable) B-1, the Applicant shall send the application via email to the
Dean of his/her Law School. After completing Section C, the Dean shall email the completed form to the Applicant, and shall
also email a copy to the Supervising Attorney listed in Section B, as well as the Administrative Office of the Illinois Courts
(AOIC) at Rule711@illinoiscourts.gov, and the MCLE Board at Rule711@mcleboard.org. A completed, signed form shall
serve as proof of the Applicant’s authorization to perform legal services pursuant to Supreme Court Rule 711(g).

I have been an active member in good standing with the Illinois Attorney Registration and Disciplinary
Commission or an equivalent authority in another U.S. jurisdiction for the previous five years and have
been an active member in good standing with the Illinois Attorney Registration and Disciplinary
Commission for the previous two years.

I will not directly supervise more than two law school graduates at one time pursuant to Illinois
Supreme Court Rule 711(g).
I will assume professional responsibility for the Applicant, and in particular acknowledge my
responsibilities as a supervising attorney pursuant to Illinois Rules of Professional Conduct 5.1
(regarding the responsibilities of partners, managers, and supervisory lawyers) and 5.3 (regarding the
supervision of nonlawyer assistants), which I have read.
I acknowledge that my participation in a Continuing Legal Education (CLE) course addressing topics
related to the supervision of lawyers and non-lawyer personnel before or during my supervision of the
Applicant is strongly encouraged, and I am aware that such CLE courses are available to me free of

Court Rule 711.
Pursuant to Supreme Court Rule 711(g)(13), I agree to collect and report data on the implementation
