ACADEMIC PETITION FORM
Student petitions for exceptions to academic policies and regulations based on compelling reasons may
be addressed to the Provost/VP for Academic Affairs. A completed petition must be submitted in
advance of the particular requirement or deadline change requested. The report of the action on the
petition is forwarded to the student, with copies filed in the student’s folder in the Office of the
Registrar. This form is not to be used for petition to change a grade nor college withdrawal.
