Completion and submission of this form is required for admission to the Cardiovascular Perfusion program.
The maximum score on the checklist is 100. The checklist counts for 15% of the admission score (35% is GPA and 50% is CASPer results).
A. Clinical Experience
Maximum Score of 50
1. Experience operating Extracorporeal Membrane
Oxygenation (ECMO). (e.g. RRT or RN, or MD with
clinical experience in Cardiovascular Care (ECMO
specialist) OR Cardiovascular Perfusionist.
40 points
Proof of registration as an RRT, RN, or MD;
Current resume and official letter from employer
verifying ECMO and/or Critical Care experience;;
Cardiovascular Perfusionist credentials.
2. RRT,RN, or MD with 2 years critical care experience
(i.e.) NICU, ICU, Operating Room.
30 points
Proof of registration as an RRT, RN, or MD. Resume and
official letter from employer detailing Critical Care
experience.
3. RRT,RN, or MD, or Allied Healthcare Worker with non-
critical care experience (i.e. Catheterization Lab)
20 points
Proof of registration as a Health Care Professional. Resume/ official
letter from employer detailing Non-Critical Care experience.
4. Authorized visit to a Perfusion Department verifying
role, responsibility and work environment for a
Cardiovascular Perfusionist.
10 points
Letter from hospital verifying visit.
B. Post-Graduate Education
Maximum Score of 30 (One Credential Only)
1. PhD in Science 30 points
Official transcripts* showing degree conferral.
2. PhD, non-science 20 points
Official transcripts* showing degree conferral.
3. Master of Science Degree 20 points
Official transcripts* showing degree conferral.
4. Master’s degree, non-science 10 points
Official transcripts* showing degree conferral.
*WES/ICAS evaluation required for international transcripts.
C. Post-Graduate Research Experience
Maximum Score of 10
1. Research paper in Human Physiology, Applied Physics,
Pharmacology, Organic Chemistry or Cardiovascular
10 points
A copy (or a link to the location) of a published peer-reviewed
academic-clinical journal.
D. Volunteer or Work Experience (within last 3 years)
Maximum Score of 10
1. Hands-on patient care. 10 points
2. Non hands-on patient care. 5 points
Detailed letter from supervisor.
