INSTRUCTIONS FOR PROJECT REVIEW FORM
Please attach additional sheets whenever necessary when space in the boxes is not adequate to provide complete answers.
A. CONTACT INFORMATION
Project Name - Please title your project to help with tracking.
Project Priority – Check the box if the project is supported by a Tribal Council Directive or if it is a Tribal priority. Please attach
supporting documentation (resolution, court order, federal directive, or Council meeting minutes with motion, etc.).
Primary Contact’s Name, Address, Phone Number, and Email Address - Provide the contact information necessary that will allow
staff from the Bad River Natural Resources Department (NRD) to contact the person who is proposing the project.
Secondary Contact’s Name, Address, Phone Number, and Email Address - Provide the contact information necessary that will allow
NRD staff to contact someone else involved with your project.
B. BASIC PROJECT INFORMATION
Proposed Start/Estimate End Dates - Provide the dates the project would ideally commence and end; if the project has already
started, then provide the date the work commenced.
Type of Project - Indicate the main purpose of the project here. For example, if your project is a new house, you need only to check
residential (then check “new” for the Scope of Project) and the assumption will be made that it will be a new build with associated
infrastructure (driveway, septic system, well, and electrical). If there are already some features on the site, like an existing driveway,
you can note that a driveway is not needed in the Additional Infrastructure Needed section.
Scope of Project – Indicate whether the project is a maintenance, expansion/addition, new, demolition, or replacement project. A
maintenance project is considered minor changes to an existing structure, an expansion/addition is considered enlarging the
size/footprint of an existing structure, a new project is constructing a structure where there is not currently one, a demolition is
removing (but not replacing) a structure, and a replacement is removing an old structure and constructing a new one in the same
location.
Project Address - This should be the fire number and street where your project is physically located. If a fire number has not been
assigned to the site yet—since you may be filling this out for a site you are interested in but don’t have yet—then fill out the
“Directions to Project Location” box as completely as possible.
Directions to Project Location - Provide directions to your project site even if you provided a “Project Address”. Directions should be
complete enough for NRD staff to locate your site both physically and on a map. If known, provide the legal description including:
Quarter, Section, Township, and Range.
Land Type - There are several different types of lands within the Reservation; some parcels are owned by the Tribe (tribal fee), some
are owned jointly or individually by tribal members and are in Trust (allotments), and some are owned privately by tribal members
and nonmembers. If you don’t know which type of land on which your project is located or that you are looking at for locating at,
contact the NRD’s Geographic Information Systems Specialist at 715-682-7123 ext. 1559 or at gspec@badriver-nsn.gov.
Lease Status - If you are planning a project on land that has already been leased to you, then check “Acquired”. If you are filling out
this form to find out if a parcel you wish to lease is a good parcel for the project you have in mind, then check “Needed”. If you are a
private landowner, then check “Not applicable”.
Plans - Indicate whether you have attached site plans or maps for submittal. Plans or maps can be hand-drawn but should include
distances/dimensions when known and as many site details as possible (location of existing buildings, forested areas, water
resources, etc.). Tribal Web Maps, which may be helpful in creating or depicting a site plan, can be accessed at http://badriver-

Project Description - Provide a narrative description of the proposed project, including any details about the size and type of
structures you may be building on the site.
Additional Infrastructure Needed - Provide details about any utilities, access roads, etcetera that may be needed to complete your
project.
FORM SUBMITTAL INSTRUCTIONS
Please submit your completed form to the Administrative Assistant at the Bad River Natural Resources Department (BRNRD). The
form can be dropped off in person at the BRNRD Office in the Chief Blackbird Center (72862 Maple Street, Odanah WI), mail it
