2020-21 RCTC FOUNDATION SCHOLARSHIP ACCEPTANCE FORM
Congratulations! The RCTC Foundation is pleased to award you a scholarship for the 2020-21 academic year. If you intend to accept this award, you
must complete this form by checking the appropriate responses, sign where indicated, and return to the RCTC Foundation Office by April 30, 2020.
RCTC Foundation Scholarship Policies
INSTRUCTIONS: To accept your scholarship award, read this form carefully, sign and return it to the RCTC Foundation. The
following items are mandatory requirements for a scholarship. Awardees who do not comply with one or more of these
requirements will have the scholarship award rescinded.
1. If a scholarship recipient defers registering for Fall semester after receiving a scholarship, he/she must notify the RCTC
Foundation of his/her intent by August 8, 2020. Please note that some scholarships are forfeited if students do not attend
Fall semester. If an awarded scholarship is not accepted or disbursed, it will be the option of the RCTC Foundation to award
the scholarship to another candidate, render the scholarship available for application to the current student population, or
cancel the awarding of the scholarship in its entirety.
2. You are responsible for providing the RCTC Foundation with a copy of your course schedule that includes your name and ID number
by August 8, 2020.
3. In most cases, scholarship criterion requires recipients to be registered as full-time students. Most scholarships can be
modified for part-time students by pro-rating the award based on credit load. A scholarship recipient who drops below full-
time status after receiving the scholarship disbursement will risk losing the full amount in the following semester.
4. Scholarship recipients are required to continue meeting the scholarship eligibility criteria when the scholarship is disbursed.
For example, a change in major, a significant drop in GPA, or change in financial aid status may disqualify the student for
the award, depending on the scholarship criteria. It is the recipient’s responsibility to be familiar with the scholarship
requirements.
5. Recipients are required to write a thank you letter for their scholarship, describing what the scholarship means to them.
It should be addressed “Dear RCTC Foundation Scholarship Donor” and sent to the RCTC Foundation office. Please mail or
email it along with this form by April 30, 2020

all requirements stated above. I also authorize Rochester Community and Technical College to release my name, scholarship information
and biographical information, including course of study and year in school, to the public through newspapers, publications and social
media for the purposes of student achievement announcements. I also give permission to the RCTC Foundation to use information
regarding my scholarship and any photos of me (which I provide or that are taken at events) to promote RCTC Foundation Scholarships.
I am choosing to decline the scholarship. By checking this box I understand that I am forfeiting this opportunity and the award will be
made to another eligible student.
