accuracy of the information provided on the application. This form is used to verify the number of members listed in either your household if you
are an independent student OR in your parents’ household if you are a dependent student.

• If your parent is remarried, include the stepparent you reported on your FAFSA or CADAA.
• If your parents are unmarried but live together, list Parent #1 and Parent #2 as you
reported on your FAFSA or CADAA.
3. Include your parent(s)’ other children (under 24) if your parents provide at least 51% of
their support between July 1, 2020 and June 30, 2021 as reported on your FAFSA or CADAA.
Family member instructions:

2. Include your spouse if you are married.
3. Include your children (under 24) ONLY IF you will provide at least
51% of their support from July 1, 2020 to June 30, 2021.
Support Instructions:
• Do not include child(ren) on this form, if you are including child support being paid on their behalf.
• You may include other dependents and/or other people who now live in the household, are being provided with at least 51% of their support, and will continue
to do so between July 1, 2020 and June 30, 2021.
A signed statement must be included explaining the reason and extent of support provided to the individual. *
