Instructions:
This application form is for the awards with a deadline of November 1, 2020 (Fall Term).
Carefully review the award description and requirements provided on the Student Awards website.
Application deadline is indicated on the Award website.
Steps to complete the award application form: (Please read it carefully)
1. Download the Adobe Acrobat Reader:
You will need the Adobe Acrobat Reader to complete the award application form with your
information.
If you dont have this free software, download it from: https://get.adobe.com/reader/
The software is compatible with Mac and PC.
2. Download the PDF award application form on your computer:
Open the application form and Save asLast Name, First Name 2020Fall.pdf” (in PDF Format)
format before entering your information into the fields.
Complete the form by entering the information directly into the fields.
Complete all sections of this application. If numeric fields do not apply, indicate “0”. If text
fields do not apply, indicate “not applicable”.
Insufficient information and/or documentation may impact the consideration of your
application.
Remember to save the application form again with your information.
3. Email the completed application form (PDF format) and the required documentations:
Application form and all required documentations must be submitted as ONEPDF File (if
applicable) via email as an attachments to Enrolment Services (awards.uoft@utoronto.ca)
Subject line must be “2020-2021 Fall Award Application.
Paper or scanned applications will not be accepted.
Select awards where you meet the award requirements.
Each form allows you to select up to 5 awards. If you wish to be considered for more than 5 awards, you may
submit multiple forms.
It may be helpful to have the Awards website open in another window/tab while completing the application form to
