In addition to completing this form, the parent must complete the Direct PLUS Loans Master Promissory Note (MPN) before the loan can disburse.
The federal government requires Feather River College (FRC) to notify financial aid recipients of important issues regarding the use and disbursement
of financial aid by giving the parent the opportunity to authorize the use of Title IV Federal Student Aid funds.
B. Authorization to pay other charges, fees, or fines for current academic year
If you’re eligible for PLUS loans in excess of tuition and fees, you can use these funds to cover other FRC related charges
and fines. The parent (you) authorizes FRC to apply Title IV Federal Student Aid funds for non-institutional charges and
miscellaneous fines (e.g. parking fines, library fines, etc.) stemming from the current academic year.

funds to other charges on the student’s account stemming from other non-institutional charges.
☐ No Checking No means that all of the other educationally-related charges as defined above will be the
student’s responsibility.
D. Parent Certification and Signature I understand this is a voluntary authorization and is valid from the date of signing
through the end of the 2020-2021 academic year. At any time I can cancel or change it by submitting a new form to the Financial
Aid Office. I further understand that my student will be responsible for paying any outstanding FRC debts if I cancel this
authorization. The parent who requested the original PLUS loan must sign and date below. Warning! If you purposely give false or
misleading information you may be fined, sentenced to jail, or both.

C. Authorization to pay other charges, fees, or fines for prior academic years (up to $200 maximum)
Title IV Federal Student Aid funds are restricted to payment of the current academic year tuition and fees. If the student
has any prior
year charges on their account, you may authorize FRC to apply Title IV Federal Student Aid funds to pay
for these charges. However, the amount applied to prior year charges may not exceed $200, if the student has prior year
charges in excess of this amoun
t they will need to arrange for payment of any additional amount owed prior to continued
enrollment.
