2020-2021 Dependent Verification Worksheet
Federal Student Aid Programs
V1/V5
Your 20202021 Free Application for Federal Student Aid (FAFSA) was selected for review in a process called verification. The law says that before awarding Federal
Student Aid, we may ask you to confirm the information you and your parents reported on your FAFSA. To verify that you provided correct information, we will compare
your FAFSA with the information on this worksheet and with any other required documents. If there are differences, your FAFSA information may need to be corrected.
You and at least one parent must complete and sign this worksheet, attach any required documents, and submit the form and other required documents to the Office of
Student Financial Services. The office may ask for additional information. If you have questions about verification, contact the Office of Student Financial Services as soon
as possible so that your financial aid will not be delayed.

I have used the IRS Data Retrieval Tool to retrieve and transfer my 2018 IRS income information into the student’s FAFSA. Proceed to Untaxed Income below.
I have not used the IRS Data Retrieval Tool when filing the FAFSA on the web. I have attached my 2018 U.S. Tax Return Transcript to this worksheet.
Please note that a tax return transcript is not the same as your original 1040 tax filing. To request a Tax Return Transcript, online or by mail, from the Internal
Revenue Service go online to www.IRS.gov. Make sure to request the “IRS Tax Return Transcript”, not the “Tax Account Transcript”. Transcripts can be ordered
via their Automated Telephone Request service at 1-800-908-9946. If using the Paper Request Form 4506T-EZ or 4506-T, first, send the transcript to your own
address, and then turn in a copy with your verification form. Verification cannot be completed until all transcripts are in. Proceed to Untaxed Income below.
IF YOU DID NOT FILE TAXES - a non-filers statement from the IRS is required
