Unusual Enrollment History Appeal Worksheet
The U.S. Department of Education has regulations to prevent fraud and abuse in the Federal Pell Grant
Program by identifying students with unusual enrollment histories. Some students who have an unusual
enrollment history have legitimate reasons for their enrollment at multiple institutions. However, such an
enrollment history requires a review to determine whether there are valid reasons for the unusual enrollment
history. This must be resolved before you will receive Financial Aid.
Students may also be selected for this process because they received Federal Pell Grant Funding over the
last four years, but did not successfully complete the number of credits for which the Federal Pell Grant was
provided.
To clear this issue, you must have all previous transcripts (unofficial transcripts will be accepted) from all
prior colleges attended evaluated by our Student Records Department. You will not be eligible for
financial aid until this process has occurred.

For each school listed above attach a detailed statement explaining the reason for your failure to earn
academic credit at that institution while receiving Federal Pell Grant funds during the review period. Please
refer to a situation that occurred during those academic terms in which you failed to earn credit. Please
indicate what you have done to address and correct the issues associated with the unusual enrollment history.
Within your statement, please include the name of the school and academic period. You may include any third
party documentation which would support your statement. Attach relevant documentation to this appeal
worksheet.
