Your 2019-2020 Free Application for Federal Student Aid (FAFSA) has been flagged for “Unusual Enrollment History Review” by the U.S.
Department of Education because you received Federal Pell Grant funds and/or Federal Direct Loans at multiple educational institutions during the
review period: 2015-2016, 2016-2017, 2017-2018, and 2018-2019. This flag requires MSJC to review your previous enrollment, academic, and
financial aid records to determine your eligibility to receive additional Federal student aid funds.

Part II. ACADEMIC CREDIT
In determining your continued eligibility for Federal student aid, you must have earned at least one academic credit, during each of the four
award years to remain eligible for Federal student aid. Unearned Academic Credit is considered grades of F, W, WF, U, I, IP, NP, NC. If you did
not earn academic credit at an institution you attended during one of the above award years (including MSJC), provide a written statement that
explains why you failed to earn academic credit at each of the institutions where no credit was earned. Attach any additional documentation
such as medical records, accident reports, etc. which substantiate your statement.
