Federal Student Aid (FAFSA). This allowed you to complete your FAFSA as an independent student who was not
required to provide parental information.
Before the processing of your FAFSA can continue, we are required to verify your independent status. Please complete,
sign, and return this worksheet with all required documentation.
The payment of your Financial Aid is on hold until your FAFSA has been reviewed for accuracy. It is imperative
that you take immediate action to complete and submit this form and any other required documentation being
requested by the Office of Financial Aid.

Documentation from your high school or school district homeless liaison that indicates you
were an unaccompanied youth who was homeless.
At any time on or after July 1, 2012, did the director of an emergency shelter program funded
by the U.S. Department of Housing and Urban Development determine that you were an
unaccompanied youth who was homeless?
Documentation from the director of the emergency shelter that indicates you were an
unaccompanied youth who was homeless.
At any time on or after July 1, 2012, did the director of a runaway or homeless youth basic
center or transitional living program determine that you were an unaccompanied youth who
was homeless or were self-supporting and at risk of being homeless?
Documentation from the director of the runaway or homeless youth basic center or transitional
living program that indicates you were an unaccompanied youth who was homeless or were
self-supporting and at risk of being homeless.
