Students who failed the annual Satisfactory Academic Progress (SAP) assessment at the conclusion of the Spring 2020 semester
and had a COVID-19 related circumstance that impacted the assessment may complete this form to have their SAP recalculated in lieu
of submitting a standard SAP Appeal. If it is reasonably determined that a COVID-19 related circumstance impacted the
completion of courses with grades of NAD, I, and W, your SAP status will be recalculated to exclude those credits without
going through the normal SAP appeals process. Please note AC grades are not included since students are given academic credit for
such grades. Once you sign the signature page, upload it before submitting the online COVID-19 Special Grading SAP Reasonable
Determination Form.
