______________ was trained on the University of Dayton’s Children on Campus and Working with Minors Policy
(“Minors Policy”) by watching the Minors Policy Training Video on ______________. The training was taken in
association with Trainee’s desire to become an “Authorized Adult” as that term is defined in the University’s Minors Policy, so
that he or she can participate in the following program or event involving minors: __________________________
By generating this Certificate, Trainee states that he or she has watched the University of Dayton Children on Campus
and Working with Minors Policy Training video; understands the behavioral and reporting responsibilities under that Policy; and
agrees to comply with that Policy.
Trainee acknowledges that generating this Certificate of Completion dishonestly – that is, without having watched the
Minors Policy Training Video, or by purposefully inserting an incorrect Trainee name or date – is not condoned by the University
and is considered a violation of the Minors Policy and may be considered a violation of other University policies or standards of
conduct. As such, dishonest generation of this Certificate could be subject to disciplinary action by the University, and/or the loss
of the privilege of being involved in University programs where minors are expected to be present.
It is recommended that Trainee save a copy of this completed Certificate in his or her files (either paper or electronic).
